Whether you’re starting your SIL or CP journey with True Living, it is more than simply moving into a new home or starting a new community participation program – it’s the beginning of a new chapter filled with opportunity, independence, and personalised support. At True Living, we understand that every individual has unique goals, preferences, support needs, and aspirations. That’s why our onboarding process is carefully designed to ensure participants and their families feel informed, supported, and confident every step of the way.
From your very first conversation with our team, we take the time to get to know you as a person, not just a participant. We work closely with you, your family, support coordinators, allied health professionals, and existing support networks to develop a transition plan that reflects your individual needs and desired outcomes. Whether you’re moving into SIL for the first time, transitioning from hospital, or seeking a better living arrangement, our experienced team ensures the process is thoughtful, collaborative, and tailored to your pace.

Onboarding Journey
1. Getting to Know You
The journey begins with an initial meeting where our leadership team meets with the participant, family members, support coordinators, and key stakeholders. During this stage, we take the time to understand individual goals, daily routines, support requirements, interests, and preferences.
When moving into a shared home, we also carefully assess compatibility between housemates to help create positive living environments where everyone feels comfortable, respected, and supported.
2. Creating Your Personalised Transition Plan
Once we understand your needs, we work together to develop a tailored transition plan. Every transition looks different – some participants are ready to move within 4 weeks of approval, while others may benefit from a gradual introduction to their new home.
This plan outlines key milestones, support requirements, communication preferences, and strategies to ensure a smooth and successful transition.
3. Completing Agreements and Documentation
Before services commence, we work with participants and families to complete the necessary documentation, including:
- Service Agreements
- Transport Agreements (where applicable)
- Board and Lodging Agreements
- Participant information and compliance requirements
Our team guides you through each document, ensuring the process is clear and straightforward.
4. Staff Training and Relationship Building
Building trust is an important part of a successful transition. Before and during the move, our support team receives training tailored to the participant’s individual needs, often in collaboration with allied health professionals such as Occupational Therapists, Speech Pathologists, and Physiotherapists.
Support workers may also complete buddy shifts to establish rapport and familiarity before services formally begin.
5. Moving In and Commencing Support
Once the transition plan is complete, an agreed commencement date is established. Participants are welcomed into their new home with a dedicated team of support workers who provide consistency, stability, and person-centred care.
Our focus is on helping participants feel comfortable, settled, and supported as they begin this exciting new chapter.
6. Ongoing Support and Communication
Our commitment doesn’t end once a participant moves in. The Senior Support Team oversees day-to-day support while our Operations Team provides ongoing oversight to ensure services continue to meet evolving needs.
Families and key stakeholders receive regular updates, progress reports, and communication regarding participant goals, achievements, and milestones.

Funding is typically approved for participants who require significant support with daily living activities. We recommend speaking with your Support Coordinator, Local Area Coordinator (LAC), or NDIS Planner regarding eligibility.
